Self-Reporting Survey for Storm Damage

If you had flooding or storm damage as a result of the storms/tornado on August 23-25, 2023, please fill out the Self-Reporting Survey at the link below. This survey was created by the Michigan State Police in conjunction with Wayne County Department of Homeland Security and Emergency Management.

If federal aid and assistance becomes available, these surveys will be used to assess and collect data pertaining to the flooding and storm damage to support requests for assistance.
